If you need a disability parking placard in Pennsylvania, Northeast Title and Tag can help. We offer the required physical examination and take care of the paperwork—making the process as easy and convenient as possible for you.

We’ll assist you with:

• Completing the MV-145A form

• Submitting the application to PennDOT

• Receiving your placard in ~2 weeks

Once your placard is processed, you can choose to pick it up at our office or have it mailed directly to you.

Step-by-Step Process

1. Book Your Exam
Schedule a quick appointment with our licensed healthcare provider using the button below.

2. Come Prepared
Bring the items listed below with you to your appointment.

3. We Complete the Paperwork
We’ll fill out the MV-145A form during your visit and submit everything to PennDOT.

4. Receive Your Placard
Once PennDOT processes your application (usually within 2 weeks), we’ll notify you. You can then choose how to receive it.

What to Bring

To ensure we can complete everything at your appointment, please bring:

• Valid Photo ID (such as a PA Driver’s License or State ID)

• Your Completed Portion of Form MV-145A (Section A) if possible (we can help if you haven’t started)

• Any assistive devices you use (e.g., cane, walker) if applicable

• Medical documents or prescriptions (optional but helpful)
